Letter of Understanding
This agreement outlines the foundation’s mission and the discounted fees your practice agrees to charge members. Contact Provider Relations for a copy of your signed agreement.
Quick Reference Form
The reference form includes membership card examples, initial and follow-up visit fees, other agreed rates and eligibility steps. Before providing services, call Member Services to verify eligibility and check photo identification.
Provider Manual
Contact Provider Relations to request the manual, a replacement copy, the Quick Reference Form or your Special Acknowledgement Certificate.
